COMMUNITY LINK WORKER

We have an exciting opportunity for a Community Link Worker to join our expanding team. The purpose of the Community Link Worker is to work alongside clients who are impacted by homelessness, poverty or social exclusion, preparing them for healthier, happier, independent and purposeful futures.  Adopting a person-centred, outcomes focussed approach, you will link clients to a whole range of social, community and partnership support, to empower them to move forward with their lives.

 ABOUT THE ROLE

  • Permanent Contract·        
  • 25 hours per week 
  • £13,800 - £15,771 
  • Location - based in the centre of Wellingborough 

 KEY RESPONSIBILITES

  • Undertaking client needs assessments and developing client led, outcome focussed support plans

  • Proactively engaging with clients, to motivate, encourage and support them in achieving their goals, monitoring and completing regular progress reviews.

  • Liaising and effectively communicating with our partnership organisations to ensure a multiagency approach in providing wrap around client support

  • Maintaining accurate, up to date client records, ensuring that all interventions and outcomes are recorded

WHO WE ARE LOOKING FOR

This role will suit a candidate who has the experience to support vulnerable or socially excluded adults in the community.  They will have a good overall knowledge of needs relating to mental health, substance misuse, benefits and housing / homelessness. They will possess excellent interpersonal and advocacy skills and an ability to relate to people in a non-judgmental way.  Their approach will be proactive, creative and solution focussed.
